CN IV Pathology
Fracture of the sphenoid wing, intracranial hemorrhage, neoplasm or aneurysm may result in injury which presents as extorsion of the ipsilateral eye. Individuals with this condition complain of worsened diplopia with attempted downgaze, but improved diplopia with head tilted to contralateral side.
