Faculty Appointment and Re-Appointment Procedure

Thank you for your interest in joining our department.  The requirements for appointment, re-appointment and/or promotion are outlined below.

NOTE: All appointments at the level of Associate Professor and above will require an interview with the Chair of Radiology, and will be reviewed by an internal Promotions Committee.
New Appointment or Renewal of Appointment:
(1) an updated CV
(2) a short letter requesting a USUHS appointment.

The letter should specify:
  • Why an appointment is desired;
  • Your qualifications and how you will enhance USUHS (e.g. How your area of clinical and/or research expertise augments USUHS);
  • How your academic efforts will support or continue to support the mission of the USUHS Department of Radiology.
For example: "I will teach USUHS students for 3 hrs/month." and/or "I will use the USUHS byline for all of my publications." and/or "I am actively involved in Residency training at ..."  etc.

A copy of the departmental strategic plan which outlines the various missions of the Department of Radiology can be obtained upon your request.  Discussions with the Chair will also aid in the delineation of opportunities for your involvement in departmental activities.
 

Renewal for Existing Appointment:
(1) an updated CV
(2) Completion of faculty re-appointment form

For Associate Professor and Professor
(New Appointments and Promotions Only):

(1) An updated, complete CV

(2) An abbreviated 2 page CV (ex. USU Board of Regents format)

(3) A letter indicating your request which also specifies your academic pathway.
The four non-tenure USUHS tracks are:

  • Clinician-Investigator Pathway (emphasizing original research and competitive funding)
  • Clinician-Educator Pathway
  • Research Pathway
  • Educator Pathway(Primary emphasis is teaching; this is only available at the Associate Professor rank)


(4) 3-5 names of reference
Actual solicitation of letters must be requested by the USUHS departmental chair and not directly by the candidate. No more than 2 local references should be from individuals at or above the requested academic rank.

(5) A Synopsis of Scholarly Interests - a brief summary of your accomplishments and future plans vis a vis the USUHS categories of:

  • Scholarship of Discovery 
    (e.g. publications, book chapters, presentations, grants and funding, IRB protocols, etc.)
  • Scholarship of Teaching 
    (e.g. medical students, residents, postgraduate, etc.)
  • Scholarship of Application 
    (e.g. offices or committee memberships in National/International societies and organizations, Editorial Board memberships, etc.)
  • Institutional Citizenship 
    (e.g. activities in direct support of USUHS, SOM, Departmental and institutional committees and/or offices held)


(6) Reprints of your 5 most significant publications

Additional Comments:

By default, all faculty not billed at USUHS will be assigned an adjunct appointment  (e.g. Adjunct Assistant Professor or Adjunct Associate Professor).  However, applicants or faculty members who demonstrate significant and continued participation in USUHS Department of Radiology activities (i.e. medical school), at a USUHS teaching hospital, are eligible for non-prefixed appointments (e.g. assistant professor and associate professor).  Individuals with prefixed (i.e. "adjunct") appointments can obtain additional information on how to increase their involvement in departmental activities by contacting the departmental chair.

Also, note that all non-billeted academic appointments are reviewed on an annual basis and that renewal (reappointment) is not automatic and requires documentation of continued activity in support of USUHS.
